Ormiston Senior College now welcomes enrolments from Year 11 and 12 students living in the enrolment zone.
Please complete the following attachments/forms and return them into the college. The enrolment process is not complete until we have received:
Enrolment application (The cover letter provides information for students and parents)
Medical Form
Parent survey/questionnaire
Student co-curricular survey
Blanket consent form
We also require:
Proof of Residence = Current phone or power account in the name of the caregiver showing the in zone address
Current passort or New Zealand birth certificate
